University of Colorado
Department of Family Medicine
We will be a leading source of innovative tools, support, education, evaluation, and research designed to improve healthcare practices, systems, and communities of health.
We are a team of clinicians, researchers, health information technology experts, practice facilitation experts, and program staff members from the University of Colorado, Department of Family Medicine
We know that good health requires effective healthcare teams connected to engaged patients and strong communities.
The Practice Innovation Program at CU is improving the quality of health and healthcare.
We work with healthcare practice teams to identify opportunities to improve the quality of care for their patients.
We convene and connect people who care about improving health in their communities.
We engage innovative practice teams to participate in timely and relevant research.
We provide support through building blocks, tools, workforce training, and education needed to drive positive change.
We collect and use data to make critical decisions.
We rigorously evaluate and research new evidence, package the best recommendations, and distribute them to our network.
The result is better health and healthcare for Colorado and beyond.
The program, under the direction of Drs. Perry Dickinson and Kyle Knierim, is focused on bringing effective practice innovation opportunities to all of Colorado’s healthcare practices.
Practice Innovation Program at CU coordinates the Colorado Health Extension System (CHES), which convenes numerous Practice Transformation Organizations across the state. These organizations deploy Regional Health Connectors (RHCs) in communities and match Practice Facilitators (PFs) and Clinical Health Information Technology Advisors (CHITAs) into practices. Together, Practice Innovation Program at CU and CHES guide learning networks that spread best practices and connect participants to local, state, and national transformation resources.
