Department of Family Medicine
The Practice Innovation Program Colorado is housed in the Department of Family Medicine at the University of Colorado School of Medicine and located on the Anshutz Medical Campus. As the convener of the Colorado Health Extension System, we mobilize quality improvement expertise from organizations across the state to engage practices and communities in achieving better population health, lower costs, and an improved experience of care for patients, families and the healthcare team.
The program, under the direction of Drs. Perry Dickinson and Kyle Knierim, is focused on bringing effective practice innovation opportunities to all of Colorado’s healthcare practices.
Practice Innovation Program Colorado coordinates the Colorado Health Extension System (CHES), which convenes numerous Practice Transformation Organizations across the state. These organizations deploy Regional Health Connectors (RHCs) in communities and match Practice Facilitators (PFs) and Clinical Health Information Technology Advisors (CHITAs) into practices. Together, Practice Innovation Program Colorado and CHES guide learning networks that spread best practices and connect participants to local, state, and national transformation resources.
With CHES we form the integral foundation of three health system improvement initiatives going on in Colorado and New Mexico right now- EvidenceNow Southwest (ENSW), the Colorado State Innovation Model (SIM) and the Transforming Clinical Practice Initiative (TCPi). Each initiative brings support to communities and practices looking to improve the health of their patients and families.
