What Is An Exemplar Practice?

Exemplar practices are in Phase 4, Phase 5, have completed Phase 5 or have completed 85% of their total possible Practice Assessment (PAT) score. They must also have demonstrated improvement in clinical outcomes and cost and utilization and have implemented 2 out of the 6 patient and family engagement metrics. With the practice’s permission, exemplar practices will be recognized and submitted to the national TCPi program’s exemplar practice registry.

A brief overview of the process is as follows:

    • Each folder contains a prepopulated template
      • The folder is also an appropriate location to upload supplemental documentation that does not fit well into the template.
    • PFs and CHITAs will only see the practices they work with on Egnyte
      • If you are new to Egnyte, you should have received an email on Tuesday 11/20 to create an account. If you did not receive an email from Egnyte to create a user account, please email tcpi@ucdenver.edu
    • PF – Value proposition, etc.
    • CHITA – CQM section
    • DFM – Cost & Utilization section – ongoing completion

The attached instructions provide additional resources and details

    • There is one task per role per practice. So while the PF can complete their section and corresponding task at a different time, the submission will not be finalized until the CHITA also completes their section and task (or vice versa, if the CHITA completes first).
    • While not all practices meet this minimum exemplar phase criteria, templates have been generated for all practices so that teams can work on them as practices continue to progress
