Stephanie Kirchner joined the Department of Family Medicine in May of 2013 as the Practice Transformation Program Manager. Stephanie has been working in quality improvement, and practice transformation in primary care since 2006, supporting practices in patient-centered medical home (PCMH) initiatives including Enhancing Practice Improving Care initiative, PCMH Foundations, the Comprehensive Primary Care initiative and Advancing Care Together. As Practice Transformation Program Manager for the Department of Family Medicine, Stephanie collaborates with project partners and stakeholders to advance practice transformation efforts across multiple initiatives, including the State Innovation Model, EvidenceNow Southwest and the Transforming Clinical Practice Initiative. Her work supports the development of practice teams that adopt sustainable, evidence-based, patient-centered quality improvement methods to achieve improved patient experience, improved patient outcomes and decreased costs of care.
