EvidenceNow Southwest (ENSW) Modules: these modules were developed as part of a grant funded by the Agency for Healthcare Research and Quality (AHRQ) with the goal to integrate the latest information and evidence on cardiovascular disease and patient engagement. State Innovation Model (SIM) e-Learning: the goal of these modules is to support the objectives of SIM to integrate mental and behavioral health into primary care. Access to these modules is limited to practices in Colorado participating in the SIM project.
Practice Transformation Organization RFP Information
The University of Colorado is issuing this request for proposal (RFP) seeking competitive, responsive applications from qualified practice transformation organizations (PTOs) to provide practice facilitation and/or clinical health information technology advisor (CHITA) services for large scale practice transformation programs. Statewide practice transformation efforts include but are not limited to the State Innovation Model (SIM) program and the Transforming Clinical Practice Initiative (TCPI). The University will select PTOs who will work together to provide practice transformation support to primary care and potentially to specialty practices to improve health and healthcare in Colorado.
The RFP consists of the following documents:
- General Information: Project Background, Overview and Goals (Informational only, nothing needs to be completed or returned)
- Attachment A: Request for Proposal (RFP) Schedule of Activities and Program Background (Informational only, nothing needs to be completed or returned)
- Attachment B: Request for Proposal Response Form (needs to be completed and returned)
- Administrative Information: (Informational only, nothing needs to be completed or returned)
DUE DATE: 3/31/2016, 5:00p.m.
Complete applications should be returned in a pdf format and sent to Natalie.buys@ucdenver.edu. Any questions should also be directed to Natalie Buys.
